Those type of label plates can be found online and often at scrapbooking stores. The labels themselves can be made with just cardstock and stained with walnut ink crystals or brewed tea bags. You could also make your own shelf similar to this style, on a larger scale, with an Expedit shelving unit from IKEA.
It’s a Pottery Barn furniture piece. I believe the metal plates come with the fixture, but I would guess that they hand created the labels for the photo shoot.
